Things To Bring on Your Initial Visit:
- A completed copy of the Privacy Practices form that you downloaded (1st page only)
- A completed copy of the Health Survey forms that you downloaded
- The actual MRI/CT images on film (or CD) along with the image report
- Please have your referring doctor fax your medical records and test results to (714) 456-8212
